Lithocholic Acid 3-Sulfate is a human metabolite, a derivative of the secondary bile acid (BA) lithocholic acid. 

Lithocholic Acid itself is a secondary BA, formed as a product of the intestinal bacteria metabolism.

Bile acids are naturally occurring steroid amphipathic molecules, formed in the liver hepatocytes as end products of cholesterol catabolism. They undergo an enterohepatic circuit, exist as anions at physiological pH, and have various properties. 

BAs main role in the human body is to aid the digestion, absorption, transport, and excretion, of dietary fats, sterols, and fat-soluble vitamins. Recently, BAs have been reported to have some additional effects such as serving as signaling molecules and participating in the regulation of cholesterol, glucose, energy, and their own metabolism and homeostasis. Alongside these effects, crucial to various physiological processes, BA abnormal accumulation results in strong cytotoxicity.

Lithocholic Acid 3-Sulfate is obtained in the bile duct, during the process of sulfation, which is generally a minor metabolic pathway. However, in the presence of any pathological conditions, such as cholestasis - elevated concentrations and reduced excretion of BAs, the fraction of sulfated BAs, including Lithocholic Acid 3-Sulfate, rises. Sulfation increases the hydro solubility of accumulated BAs, easing their renal clearance. Thus, Lithocholic Acid 3-Sulfate is part of an important protective metabolic mechanism to limit BAs accumulation and tissue damage.

The sulfated derivative is also found to increase in models of streptozotocin-induced rat diabetes. Streptozotocin is a naturally occurring antineoplastic agent. However, it is toxic to insulin-producing pancreatic beta cells.

In addition, Lithocholic Acid 3-Sulfate is believed to be involved in the pathogenesis of pancreatitis.
