Sitostanol is a plant sterol that belongs to the same family as campesterol and beta-sitosterol. Sitostanol, especially fat-dissolved plant stanol esters, can be taken as a dietary plant sterol, which can decrease serum total and LDL cholesterol as levels reflect cholesterol absorption.

In one study, the replacement of a portion of daily fat intake with sitostanol-ester margarine proved to be effective in lowering serum total cholesterol and LDL cholesterol levels in a mildly hypercholesterolemic population.

Its relevance extends to conditions such as non-alcoholic fatty liver disease (NAFLD), where the sitostanol-to-cholesterol ratio is implicated and correlates with liver steatosis and inflammation.
