3β,5α,6β-Trihydroxycholanoic Acid-D4 is a human metabolite, a bile acid (BA), and it is obtained from another BA - cholanoic acid/cholanic acid, an archetypal C24 BA, from which other C24 BAs and more can be synthesized.

Corresponding to the BA 3β,5α,6β-Trihydroxycholanoic Acid-D4 is the compound cholestane-3β,5α,6β-triol (3β,5α,6β-triol). It belongs to the group of oxysterols which are usually present at minor concentrations in human samples. However, their levels get significantly elevated in some specific diseases, particularly inborn errors of sterol biosynthesis, metabolism, transport display, and non physiological metabolic pathways, that become major processes under pathological conditions, but minor in a healthy state. Examples of such diseases are lysosomal storage disorders, such as Niemann-Pick disease type B (NPB) and type C (NPC), Smith-Lemli-Opitz syndrome, cerebrotendinous xanthomatosis, and more. They occur as a result of different gene mutations and cause increased oxysterol synthesis.

The way cholestane-3β,5α,6β-triol is further metabolized has been of great interest recently. Not only are its metabolites studied to have a great role in embryonic development, but cholestane is also metabolized to its relative BA, the compound 3β,5α,6β-Trihydroxycholanoic Acid-D4, which has been identified as a diagnostic marker in plasma of congenital metabolic disorders, and more.

Additionally, 3β,5α,6β-Trihydroxycholanoic Acid-D4 is linked to derivative metabolites with anti-tumor effects, such as dendrogenin A (DDA). In some specific cancers, like breast tumors, DDA is found to be decreased, compared to healthy tissues - proof that sterol metabolism is connected to neoplastic growth.
